Day School Student Photo and Video Consent

At Crossroads Continuum, we use a variety of media to promote our programs, educate the community, and communicate with families and donors. Often, video and pictures are important for telling our story and capturing the energy of our programs.

Please complete the form below for enrolled students to provide consent for them to be included in photos and videos. Use student name for first and last, you may you use your email address if they do not have an email address.
