Center-Based adult services for ages 22+

Much like a Town Commons, Crossroads Continuum Commons brings individuals together and celebrates community. Our program is designed to maximize each individual’s independence and enrich their life through a range of off-site activities. The program uses a person-centered approach to develop, enhance, or maintain skills that allow each individual to thrive in their community.

  • Career exploration through volunteer experiences, internships, or situational assessments.

  • Community Based Day Service activities promote integration and inclusion into the community daily. Individuals in the program will participate in a range of activities over the week that include volunteer opportunities, employment exploration, skill development activities and leisure outings.

  • The program is led by an experiences Program Manager who is a Board-Certified Behavior Analyst (BCBA) and a Licensed Applied Behavior Analyst (LABA). The program employs several skilled trainers who provide hands-on support and skill-development. All staff are trained in Positive Behavior Supports, Autism and related disabilities, human rights, and communication and social skills to name a few.

  • Referrals are received through the Department of Developmental Services (DDS). The agency also accepts private pay.

  • Office Hours are Monday to Friday, from 8:00 AM -4:00 PM

    Program Hours are Monday to Friday from 9:00 AM – 3:00 PM

Support our Commons Program

State funding for adult services is approximately 1/3 of funding for school-based programs. This means the generous support of our community and local businesses is essential to offering the same high-quality staff, learning spaces, and curriculum the individuals and families we serve have come to expect.
